目的

探讨牙周炎及吸烟单因素和双因素协同效应对种植体周围牙槽骨的影响。

方法

设置健康组、牙周炎组、吸烟组、牙周炎合并吸烟组4 个后牙缺失病例组,植入Straumann 纯钛螺纹柱状标准型种植体进行修复, 通过SIDEXIS-XG 软件测算负载后1 年种植体边缘牙槽骨吸收状况(MBL);记录负载12 个月时改良菌斑指数(mPLI)、改良出血指数(mSBI)、探诊深度(PD);进行统计学分析。

结果

牙周炎组与健康对照组的各项指标差异无统计学意义(P >0.05),吸烟组与健康对照组的各项指标差异也无统计学意义(P >0.05),但牙周炎合并吸烟组与健康对照组在MBI 及mSBI 的差异有统计学意义 (P <0.05),在mPLI 及PD 的差异无统计学意义(P >0.05)。

结论

积极有效的牙周治疗及维护下,牙周炎或吸烟者植入Straumann 标准型种植体,可获可靠疗效,但对牙周炎合并吸烟者可出现边缘骨明显吸收,更容易出现种植体周探诊出血,在病例选择上要引以注意。

Objective

To investigate the effects of periodontitis and smoking by single factor and two-factor synergistic action on the peri-implant alveolar bone loss.

Methods

Four groups of posterior partially edentulous patiens were established: healthy control group;periodontitis group; smoking group; periodontitis combined with smoking group. ITI implants were embedding into the jaws of patiens in four groups seperately. X-ray examinations were made and implant marginal alveolar bone loss (MBL) were measured 12 months after loading with the SIDEXIS-XG software. Besides, 12 months after loading, improved plaque index (mPLI),modified bleeding index (mSBI), probing depth (PD) were also recorded. SPSS 12.0 was applied to analyze the data.

Results

There were no significant difference between periodontitis group and healthy control group (P>0.05). Simultaneously, no significant difference between smoking group and healthy control group (P >0.05). There were, however, significant difference in the MBI and mSBI (P <0.05), no significant difference (P >0.05) in the mPLI and PD between smoking combined with periodontitis group and healthy control group.

Conclusions

Reliable effects can be achieved when ITI implant system is applied in periodontitis patiens with effective periodontal treatments and maintenances. However, it may appear obvious marginal bone resorption in periodontitis plus smoking group, which should be careful during cases choosing.
