Submandibular gland excision by intraoral approach

Abstract:

Objective

To investigate the feasibility and safety of intraoral submandibular gland excision.

Methods

Analyze the relationship between external maxillary artery and submandibular gland by dissecting 70 cases and reviewing the literatures so as to offer a reliable anatomical base for the 15 cases of intraoral submandibular gland excision including 2 cases with the aid of endoscope.

Results

67.1%(47/70) of the external maxillary artery freaded on the surface of submandibular gland, providing nutrition for the gland through 2~4 branches. The processes of 15 cases of intraoral submandibular gland excision were smooth, and the results of all the cases were effective and satisfying, without massive hemorrhage during or post-operation. External maxillary artery needed to be ligated only in 4 cases of intraoperation. Endoscope and ultrasound knife facilitate the ligation and hemostasis of vascular vessels.

Conclusion

Intraoral submandibular gland excision is safe and feasible as long as indications are strictly controlled and the external maxillary artery is well coped with.Endoscope improves the operating vision in deep region.
