目的
探讨下颌第二磨牙半切术后不同修复方式对基牙受力的影响, 为临床修复设计提供依据。
方法
用三维有限元分析方法,分别模拟近中根拔除、远中根保留和远中根拔除、近中根保留后固定桥修复,同时建立联冠修复和缺失单基牙固定桥修复作为对照,比较载荷状态下基牙的受力状况。
结果
保留远中根、固定修复与正常情况下受力趋势相似,应力值较大;保留近中根、单端固定桥修复应力明显增大且峰值出现部位由颈部转移到根尖,但较缺失、以为基牙的单端固定桥受力合理。
结论
保留下颌第二磨牙远中根较保留近中根与第一磨牙联合修复后基牙受力合理;只要选择好适应症,保留近中根也是避免下颌第二磨牙缺失的一种方法。
Objective
To evaluate the mechanical behavior of the abutment teeth of the fixed restoration after the mandibular second molar being sectioned.
Methods
Finite element method was employed to investigate the stress distribution around the reserved distal or mesial root of the second mandibular molar after restored with fixed prosthesis of various designs.The model of combined crowns of and missing restored with supported cantilever fixed bridge were designed as control.The same oblique load was applied on the occlusal surfaces of the molars,the stress distribution was analyzed.
Results
The stress status of the reserved distal root of combined is similar to the combined crowns of , with the stress intensity slightly higher;The stress around the reserved mesial root of combined is significantly higher than the combined crowns of , the stress concentration exhibited in the apical regions, instead of in the cervical regions, but the stress distribution was more favorable than supported cantilever fixed bridge.
Conclusion
For the various prosthetic designs for hemi-sectioned , the stress status of the fixed prosthesis reserving the distal root of is more favorable than that of reserved mesial root of .However, that latter is also an alternative to avoid missing , on condition that the indication is strictly commanded.
