目的

探讨单侧颧骨骨折在手术治疗中既保持面部美观,又能达到良好复位固定的方法。

方法

对17 例患者行上颌前庭沟切口+颧骨颞骨头钢丝牵引复位固定后,在术后1 个月、3 个月、6 个月、1 年时行临床检查,3 个月时加作计算机体层摄影术(CT)检查及定位X 线片头影测量双侧外侧缘高度、眶下缘宽度、双侧颧骨长度,并对计量资料作t 检验。

结果

所有患者术后软组织均Ⅰ期愈合,术后1 年内定期复查未见明显复发,复位颧骨无移位、变形现象,3 个月时CT 示颧骨对位对线良好,头影X 线片测量分析:双侧眶外侧缘高度、双侧眶下缘宽度、双侧颧弓长度比较差异均无统计学意义(P>0.05)。

结论

单侧简单颧骨骨折手术进路与固定术式改进方法,是一种在术后面部不留瘢痕、手术复位与固定效果稳定、功能修复良好的手术方法,值得推广应用。

Objective

To investigate the surgical method of simple unilateral zygomatic fracture which can keep facial bisymmetry and excellent stability of reduction and fixation.

Methods

17 cases with zygomatic fracture were included in this study. The patients were given maxillary vestibular sulcus incison and zygomatic temporal bone head traction fixation. Clinical examinations were followed up at 1 month, 3 months, 6 months and 1 year. CT test and cephalometric analysis X-ray measurement were taken at 3 months after surgery to achieve the height of the bilateral exterior orbital rim, the width of the bilateral inferior orbital rim and the length of the bilateral zygomatic arch. Then datas was analyzed with t test.

Results

The soft tissue of all 17 cases healed primarily. No obvious recurrence such as displacement or deformation of the zygoma was found one year post operation. CT and cephalometric analysis taken at 3 months showed that the zygoma healed well on fracture line and the t test showed no statistical difference (P>0.05).

Conclusions

This improved surgical approach and fixation method for simple unilateral zygomatic fracture is a satisfied surgical method with no facial scars, excellent stability of reduction and fixation, and good function, which is potential for further application.
