目的

研究颞下颌关节（TMJ）上腔注射透明质酸钠（SH）治疗颞下颌关节骨关节病（TMJOA）前后关节滑液中基质金属蛋白酶2（MMP-2）和MMP-3 含量的变化，探讨SH 治疗TMJOA 的机制。

方法

将30 例TMJOA 患者随机分成关节腔冲洗加SH 治疗组和单纯关节腔冲洗组两组，6 例志愿者为对照组。 观察两种方法治疗前后关节滑液中MMP-2、3 含量的变化。

结果

冲洗加SH 组的临床疗效优于单纯关节腔冲洗组（开口度比较P ＜0.01，VAS 比较P ＜0.05）；无症状志愿者中1 例检测出少量的MMP-2（31.12 μg/L）和MMP-3（13.32 μg/L）；治疗后冲洗加SH 组比单纯关节腔冲洗组MMP-2、3 含量明显降低（P ＜0.01）。

结论

SH 可能通过降低TMJOA 患者关节滑液中的MMP-2、3 水平，减缓了关节软骨基质破坏速度，从而发挥对TMJOA 的治疗作用。

Objective

To study the change of matrix metalloproteinase-2 （MMP-2） and matrix metalloproteinase-3 （MMP-3） level in temporamandibular joint （TMJ） synovial fluid after sodium hyaluronate （SH） irrigation in temporamandibular joint osteoarthritis （TMJOA） patients.

Methods

Thirty patients suffered from TMJOA were randomly devided into two groups， treated respectively with lavage and SH injection for the trail group and only lavage for the control group.The MMP-2 and MMP-3 level was examined before and after the treaments.

Results

The clinical effect of the trail group was better than that of the control group （the opening degree：P ＜0.01，VAS：P ＜0.05）； A small amount of MMP-2 （31.12 μg/L） and MMP-3 （13.32 μg/L） were detected in one case of asymptomatic volunteers； The change of MMP-2， 3 level in the trail group was higher than that in control group.

Conclusion

SH may slow the destruction of articular cartilage matrix by reducing the MMP-2， 3 level in TMJ SF in TMJOA patients， thus plays a role in the treatment of TMJOA.
