目的
评价两种根充法治疗慢性根尖周炎的术后肿痛发生率。
方法
对250 例无明显临床症状但X 线检查根尖有阴影的患牙,随机分为2 组:(1)一次性根充组129 例,通过根管预备及根管充填,一次性完成根管治疗;(2)分次根充组121 例,根管预备后氢氧化钙封药,1 周后临床检查无痛再行根管充填。 比较这两种根充方法分别在术后第2 天与术后1 周对患牙肿痛发生率的影响。
结果
根管畅通患者中,一次根充组术后第2 天及1 周的肿痛发生率分别为28.2%和8.5%,其肿痛消失时间为(1.69±0.84)d;分次根充组的肿痛发生率分别为30.6%和4.9%,其肿痛消失时间为(1.90±0.46)d。 根管狭小的患者中,一次根充组术后第2 天及1 周的肿痛发生率分别为37.9%和15.5%,其肿痛消失时间为(2.15±0.96)d;分次根充组肿痛发生率分别为28.8%和16.9%,其肿痛消失时间为(2.09±0.83)d。两种根充方法在肿痛发生率及肿痛消失时间方面差异均无统计学意义(P >0.05)。
结论
对于慢性根尖周炎而言,一次性根充法在短期内疗效与分次根充法相当,又减少了患者就诊次数,有望在临床推广使用。
Objective
To investigate the incidence of postoperative pain in patients with chronic periapical periodontitis undergoing single- versus multiple- visit root canal treatment.
Methods
A total number of 250 teeth, which were asymptomatic but had periapical radiolucent lesions were assigned randomly into either single- (129 teeth)or multiple-visit (121 teeth) groups.(1) single-visit group: root canal treatment was performed in one visit without root canal medication; (2) multi-visit group: calcium hydroxide intracanal medication was carried out after root canal preparation. Root canal obturation was performed after one week when teeth were asymptomatic. The incidences of postoperative pain on day two and day seven were recorded.
Results
For teeth with patent root canals, the incidences of postoperative pain in single-visit groupwere 28.2% and 8.5% on the second and the seventh day respectively, and the duration of pain was (1.69±0.84)d; The incidences of pain in multiple-visit group were 28.8% and 16.9%on the second and the seventh day respectively, and the duration of pain was (1.90±0.46)d. For teeth with narrow root canals, the incidences of postoperative pain in single-visit group were 37.9% and 15.5% on the second and the seventh day respectively, and the duration of pain was(2.15±0.96)d; however, the incidences of postoperative pain in multiple-visit group were 28.8%and 16.9% respectively, and the duration of pain was (2.09 ± 0.83)d. There was no significant difference in postoperative pain between single-visit and multiple-visit root canal treatment.
Conclusions
For teeth with chronic apical periodontitis, no significant difference in postoperative pain between single- and multiple-visit root canal treatment. As single-visit therapy reduces the times of visit, it may be a preferred choice in clinic.
