目的

评价负压吸引术治疗大型牙源性角化囊肿的临床疗效，探讨角化囊肿减少的机制。

方法

选取15 例大型牙源性囊肿患者，用负压吸引术治疗，对治疗前后的临床表现、影像学表现、组织病理学观查及囊壁的PCNA 和Ki-67 免疫组化染色结果进行比较。

结果

术后1～2 个月15 例患者面部畸形完全消除；术后第2 个月和第6 个月，囊肿分别减小52.8% 和89.6%；新生骨平均厚度2.6 mm、4.84 mm；术后衬里上皮Ki-67 平均细胞增殖指数（49.2／mm）比治疗前（68.8／mm）显著降低，差异有显著性（P ＜0.05），而PCNA 阳性细胞数密度在治疗前后差异无显著性。

结论

负压吸引术能迅速消除面部畸形，促进囊肿周围新骨的形成，更好地恢复面部的正常形态及功能。

Objective

To evaluate the effect of Suction Drainage （SD） in the treatment of large odontogenic keratocysts.

Methods

Fifteen patients with large odontogenic keratocysts were recruited for the study. Clinical parameters， radiographic assessment and histopathological observation were employed to evaluate the treatment effects before and after the usage of SD. Ki-67 and PCNA were performed to detect the proliferation activity of cyst epithelium cells by immunohisto-chemistry.

Result

After the usage of SD， the facial deformity of all the patients disappeared within 1 ～2 months； the size of cyst decreased about 52.8% and 89.6% at the 2^nd and the 6^th month respectively.Histopathological observation showed that the average thickness of new bone was 2.6 mm at the 2^nd month and 4.84 mm at the 6^th month. Significant decrease of Ki-67 was also found after the treatment.

Conclusion

Suction drainage may rapidly correct the facial deformity and promote the formation of new bone adjacent to the cyst lesion.
