目的

探讨重组人p53 腺病毒（rAd-p53）联合放射治疗晚期口腔癌患者的护理。

方法

对2009 年1 月至2012 年4 月在中国人民解放军总医院接受rAd-p53 注射联合放射线治疗晚期口腔癌的患者，治疗前给予心理护理及健康指导，注射过程中做好护理配合，严密观察治疗后病情变化，及时处理发热、局部肿胀、疼痛、胃肠道反应等不良反应，做好放疗后皮肤护理、口腔护理及饮食护理， 观察疗效并总结各项护理要点。

结果

22 例患者均顺利完成rAd-p53 联合放射治疗，其中部分缓解10 例（45.5%）、稳定8 例（36.4%）、进展4 例（18.1%）。 全部病例均无并发症及严重不良反应的发生。

结论

正确合理的护理是rAd-p53 瘤内注射联合常规放疗治疗晚期口腔癌的重要环节，可有效减少不良反应的发生，减轻患者痛苦，达到提高治疗效果和生活质量的目的。

Objective

To investigate the nursing care of the advanced oral cancer patients treated by the recombinant adenovirus p53 agent combined with radiotherapy.

Methods

22 patients diagnosed as late oral SQCA received extra treatment of rAd-p53 in combination with radiotherapy in PLA General Hospital from January 2009 to April 2012 were included. During the whole process，phycological nursing and health guidance were given to the patients before treatment， good nursing cooperation was made during the process of injection； After treatment， the patients' conditions were carefully observed； side effects of fever， swelling， pain and gastroenteric reaction were managed timely，and careful nursing for skin， oral cavity and food were implemented. The preoperative nursing，postoperative condition data， complication data were collected and reviewed.

Results

After preoperative health guidance， medication care， accurate observation on postoperative patient's condition， effective solution for adverse reactions and psychological nursing care， 22 cases were successfully completed， PR 45.5%， NC 36.4%， PD 18.1%， without any adverse reaction occurred.

Conclusions

Reasonable nursing care is one of the most essential parts in the treatment of oral cancer patients with combined rAd-p53 injection and radiotherapy， which can effectively improve the therapeutic effect， reduce the occurrence of adverse reaction.
